Find the first unsupported learner decision
Start with a real attempt at the promised performance and move backward until you locate the earliest decision the learner cannot make with available knowledge and support. The first visible failure may occur in the final project, but its cause may be an undefined term, an unpracticed distinction, or a tool action several lessons earlier. Diagnosing the first unsupported decision avoids adding more explanation to the wrong place.
Completion, time on page, and quiz scores can help locate friction but do not identify the mechanism. CMU’s objective guidance provides the first check: does the assessed performance match the stated learner outcome? If the objective promises application but lessons and quizzes reward recognition, successful completion can coexist with inability to act.

Branch 1: the objective and assessment ask different things
Expected trace: learners pass recall checks yet fail when they must select, explain, create, or troubleshoot. Another trace is that creators cannot agree what a successful final performance looks like. Compare the objective, examples, practice, feedback, and assessment using the same behavioral language. If different capabilities appear, alignment is the leading cause.
The repair is not automatically a harder test. Rewrite the objective or redesign earlier attempts so learners practice the promised decision. Preserve evidence of what the old sequence actually taught. A narrow, honest objective is better than a broad promise followed by unrelated assessment.

Branch 2 and 3: missing prerequisite or inert explanation
A missing prerequisite produces predictable breakdown at a dependency: unfamiliar notation, absent concept, or tool operation. An entry task or think-aloud can expose it. Inert explanation occurs when learners can repeat information but have not practiced choosing when and how to use it. The trace appears when cues are removed or the surface context changes.

Feedback failure appears when learners know an answer was wrong but not which decision or criterion needs revision, or when feedback arrives after the next attempt. Inspect whether comments identify a trace, consequence, and retry path. An access barrier appears when navigation, media, language, timing, or response format blocks participation independently of the objective.
CAST’s UDL Guidelines support multiple means of engagement, representation, and action or expression. Use that lens to test whether an alternative route preserves the same substantive performance. Do not remove legitimate assistive support to create a supposedly clean diagnostic. If performance improves when an irrelevant barrier is removed, credit accessible design rather than calling it remediation of the learner.

Build the first-breakpoint evidence trace
Create columns for pathway step, required capability, evidence offered, learner action, observed trace, competing cause, and next separating observation. Add task conditions and assistance. Walk one unsuccessful and one successful case when possible. The contrast can reveal hidden prior knowledge or support that the published path never teaches.
Illustrative example: learners complete a lesson on source quality but cannot choose evidence for a claim. One cause is missing source-role knowledge; another is that examples always label the best source. A parallel task with labels removed and a short explanation requirement separates them. Repair one link and verify changed-context use
Choose the strongest causal branch and repair only that link: add an entry route, insert a meaningful attempt, make feedback actionable, or remove an access barrier. Then retest on a comparable task and later on a changed context. Keep contradictory cases and do not attribute improvement from one attempt to the repair alone; familiarity, extra time, or support may explain it.
Some gaps require qualified instruction, supervised practice, or a changed objective. A diagnostic tree cannot guarantee learning or replace accessibility expertise. Record the decision and review by 2027-02-10, earlier after a major audience, platform, objective, or assessment change. The useful outcome may be to shorten the course or state a narrower promise.
